Telemedicine allows you to have a virtual appointment with your primary care or psychiatric provider using a digital device and the internet, where you can discuss your healthcare concerns from the comfort and safety of your home.
How Does it Work?
We will schedule a video appointment just like an in-office visit. You will need to access our free Telemedicine conferencing tool on your computer or smart device. You will be able to discuss your healthcare concerns, including your symptoms, lab work, prescriptions and more, all from the convenience of your home.
Telemedicine provides you with convenient and safe access to health care from your home, office or virtually anywhere. You can meet with a Horizon health care team member in real time using telemedicine technology that is easy to use and accessible from a variety of your personal communication devices (smartphone, laptop, desktop and tablet). However, if we determine that an in-person visit is needed or you would prefer to meet in-person, we will schedule an appointment for you at one of our clinics.

Telemedicine services are available for new and existing patients. For most cases, a Telemedicine visit allows your provider and health care team to deliver the same level of care as in our office.

How Do I Schedule an Appointment?
Patients can call our Customer Care Center at (833) 431-4449, Monday through Friday from 8 a.m. to 5 p.m. to determine if their needs can be best served with a Telemedicine appointment. Our customer service representative will assist you in scheduling an appointment.
