Horizon offers COVID-19 testing and vaccinations to help keep our communities safe and healthy.
COVID-19 Vaccinations
In our continued efforts to provide quick and efficient access to vital COVID-19 vaccines, appointments are no longer needed at Horizon Health and Wellness.
We administer Moderna COVID-19 vaccinations for adults ages 18 and older on a walk-in basis at our Apache Junction location.

The Centers for Disease Control (CDC) has expanded eligibility for a second booster shot for adults aged 50 and older. For more information, click here.
COVID-19 Booster/Third Doses
Horizon Health and Wellness will be offering COVID-19 booster doses starting Sept 20, 2021, as recommended by the CDC. However, if you are immunocompromised, you may be eligible for a third dose now. For more information on whether you qualify, please visit the CDC’s website for further recommendations on the use of boosters and third doses for the public.
COVID-19 PCR and Antibody Testing
COVID-19 PCR and Antibody testing are available by appointment only at all Horizon clinics, with the exception of our Globe location. There is no up-front cost for testing if you have insurance; however, a visit with one of our healthcare providers is required.
Two forms of testing are available at specified Horizon sites.
Diagnostic PCR Test
- Detects if you’re actively infected with the virus
- Nasal swab test
- Sent to a lab for testing and diagnosing
Antibody Testing
- Detects if you were exposed to the virus in the past
- Blood draw test
- Sent to a lab for testing and diagnosing

COVID-19 Rapid Antigen Testing
COVID-19 Rapid Antigen Testing is available by appointment only at Horizon’s Apache Junction Clinic. There is an out-of-pocket cost of $80 for the Rapid Antigen Test and insurance will not be billed. If you are using the Rapid Test results for travel, please verify that a Rapid Antigen Test is acceptable.

- Detects the presence of viral proteins (e.g., coronavirus)
- Nasal swab test
- Analyzed on-site, with results available within one hour
