Transitional Housing

Horizon Health and Wellness provides Transitional Housing for Arizona Families First participants which includes a minimum stay of 6 months. Our units provide at a minimum the following:

  • Safe, stable living accommodations
  • Services to develop skills and personal characteristics needed for independent living
  • Substance abuse education and counseling
  • Referrals and access to medical and mental health treatment
  • Employment assistance
  • Education assistance

Horizon Health and Wellness maintains safe and affordable housing units that meet all state and federal standards. These homes are monitored by Horizon Housing staff regularly to ensure compliance is maintained. Horizon has sought to change how participants achieve self-sufficiency through the utilization of supportive services. These services are delivered by direct care staff responsible for the overall coordination of individualized services. These services typically include assistance in developing living skills, money management, medication monitoring, mental health and medical appointments, shopping and many others. Participants are also educated regarding how to maintain and care for their housing in conjunction with their rental lease, repairs, and reporting requirements to ensure greater success when they transition into permanent housing.
